SM64 Halloween Mayhem is a mini ROM hack made by Kaze Emanuar and Kinopio, which was created for Halloween and released on October 2014.
This hack features eight levels, each with one Star included.

Deadly Slide

To enter this level, Mario has to take damage to fill the blood pond. Hitting the swinging axes down the slide results almost an instant death.
Haunted Manor

To enter the level, Mario has to acquire the key to open the only locked door of the mansion; that is, collecting 35 coins and drop them in the wishing well.
Cursed Egyptian Tomb

Inspired by the tomb stages in Crash Bandicoot 3: Warped, Mario has to survive obstacles and traps to collect the star.
Death O'Clock

To enter this level, Mario must find the switch hidden in the overworld to unseal the entrance of the clock tower.
Minecart Madness 64

Inspired by the minecart stages from the Donkey Kong Country series, Mario has to make to the end while jumping over gaps and rails with the cart without falling off.
Bewitched Cauldrons

Located at the shed with the smoke out of the chimney, Mario has to collect all 8 red coins spread around the boiled cauldrons.
Final Showdown

Mario must climb to the top of the before the rising lava engulfs the tower all the way up.
The Rom's Frontiers

Once all 6 stars are collected, a UFO will appear next to the farm abducting Mario to the final level.
